Home Health Aide (Personal Care & Homemaking)

Position Status: Hourly, Non-Exempt, Full-Time or Part-Time  

Reports To: Registered Nurse (RN)  

Job Description Summary  

The Home Health Aide (HHA) is an hourly, non-exempt position responsible for providing  personal care and homemaking services to patients in their residences. The HHA works under the  direction and supervision of a Registered Nurse (RN) and adheres to the policies and procedures  of Ultimate Care Services, Inc., in compliance with Ohio Administrative Code (OAC)  requirements.  

Qualifications  

1. Must be at least 18 years of age.  

2. Must be authorized to work in the United States.  

3. Must have a high school diploma or equivalent (exceptions for experience may be made)  

4. Completion of a 30 thirty hours of pre-service training acceptable to the department of  aging.  

Or 

75-hour Home Health Aide training program, including:  

o 16 hours of classroom training before client contact.  

o Supervised practical training under the direction of an RN or LPN.  

5. Successful completion of a competency evaluation as required by OAC. 

6. Exemptions may apply for:  

o State-Tested Nursing Assistants (STNAs) certified by the Ohio Department of  Health.  

o Individuals who have completed approved nursing education or aide training  programs.  

7. To maintain eligibility, each personal care aide must successfully complete 6 hours of in service training every twelve months. 

8. Must pass a criminal background check through the Bureau of Criminal Identification  and Investigation (BCII).  

9. Ability to follow written and verbal instructions.  

10. Physical ability to perform required tasks, including bending, lifting, and assisting with  mobility.  

11. Completion of annual in-service training (12 hours) to maintain qualifications.  

Responsibilities  

1. Adheres to all established agency policies and procedures.  

2. Provides personal care services, including bathing, grooming, dressing, and hygiene as  directed by the service plan.  

3. Assists with household services essential to health care in the home, including light  housekeeping, meal preparation, and laundry.  

4. Completes required visit documentation promptly and accurately.  

5. Reports changes in the patient’s condition or needs to the supervising RN immediately.  

6. Assists with mobility exercises, including ambulation, range of motion (ROM), and  transfers as directed.  

7. Supports self-administered medication as assigned in compliance with training and  policies.  

8. Participates in annual in-service training and continuing education programs.  9. Attends patient care conferences as scheduled.  

Working Environment  

• Works indoors in client homes and occasionally at the Tallmadge office for meetings and  training.  

• Travels between client residences as required.  

Risk Exposure  

High Risk: Frequent exposure to potential infectious diseases, physical strains, and  hazardous environments in client homes. 

Lifting Requirements  

The Home Health Aide must be able to:  

• Participate in physical activities required for patient care.  

• Work for extended periods while standing and performing physical tasks.  • Lift and carry up to 50 lbs regularly.  

• Perform extensive bending, lifting, and transferring tasks.  

Job Limitations  

The Home Health Aide is prohibited from performing any tasks that fall under the scope of licensed  nursing practices, including:  

1. Administering or injecting medications.  

2. Taking physician orders.  

3. Performing sterile procedures or tasks requiring advanced nursing skills.
